How does First Link work?

Not sure how First Link works? Read this step by step guide to implementing the program in your office:

  1. Once you've made a diagnosis, asks the person with dementia and his or her family if you can refer them to First Link.
  2. Once consent is given, you need to complete a referral form and send it to the First Link team at the Alzheimer Society of Thunder Bay.
  3. Within four weeks, our First Link Coordinator will call the person with dementia or a caregiver to provide information, assess needs and connect them directly to information and support services in our community. Those diagnoses and their families can also call First Link directly.

Care partners are currently the largest source of referrals. Increase in physician referrals will decrease time referral and improve health outcomes for both people with dementia and their care partners.
