The Alzheimer Society of York Region has long established and valued partnerships within the Region of York.
Our Caregiver Support and Education programs are integral aspects of these successful partnerships. Our Social Workers are embedded in our partners’ cognitively impaired adult day programs at Mackenzie Health Centre, Circle of Care, and CHATS (Rural and Balance of Care). All clients are assessed by Alzheimer Society social workers prior to admission to the respective day programs and the family receives ongoing support and education. The caregiver will also continue to receive support after their family member has left the day program service. This care throughout the continuum of the disease is beneficial for the caregiver.
We also offer family support groups in partnership with the Region of York (Maple and Keswick), Markham Public Library, and Parkview Home. Through unique partnerships with Parkview Home and the Bloomington Cove Care Community our social workers also provide support to the residents and families of these two long-term care facilities. Our First Link program is partnered with Mosiac Home Care to provide the First Link Memory Café. This Café provides persons who have been recently diagnosed with early stage dementia and their care partners with an opportunity to socialize, network and gain information about memory issues in an informal setting.
By maximizing expertise, sharing information and working collaboratively together, agencies are better able to meet the needs of persons living with dementia. The Alzheimer Society of York in collaboration with our valued partners has demonstrated that the integration of services is best practice for service delivery.
