The Alzheimer Society of Grey-Bruce is proud to offer the Kelly Graham Memorial Scholarship and Forget-Me-Not Award each year to individuals who have made a contribution to dementia care in Grey and Bruce Counties. Nominate someone for the Forget-Me-Not Award or apply for the scholarship today.
Apply for the Kelly Graham Memorial Scholarship
One $1,500 scholarship will be awarded annually to a Canadian citizen who resides or goes to school in Grey-Bruce who has shown exceptional drive in the areas of leadership, community volunteerism, and supporting individuals living with dementia and/or their caregivers in other health and social services. The deadline for applications is May 31st, 2026.

Nominate a Community Hero for the Forget‑Me‑Not Award
The Forget-Me-Not Award was established in 2019 to recognize individuals whose leadership has advanced the cause and care of Alzheimer's disease and other dementias in Grey-Bruce. Award winners will have demonstrated excellence in areas such as providing care, raising awareness, and increasing inclusion, acceptance, and understanding.
Nominees will be judged on criteria including, but not limited to:
- Collaboration with others
- Compassion and respect
- Communication skills
- Courage and resilience
- Creativity
- Expertise in the role
- Impact in community
- Initiative
Nominations will be accepted at any time of year for consideration in the Fall. To nominate, please email us at [email protected]. The nominating committee will select at least one Forget-Me-Not Award recipient per year up to a maximum of three winners.
