Social Worker

Overview
As a Social Worker you will bring your knowledge of social work to our team to make a difference in the lives of those living with dementia and their care partners. In this position, you will provide ongoing support to care partners of those living with dementia. You will help the care partner access information and health care services that allow them to care for the person living with dementia at home for as long as possible. You will provide information, education, training, and therapeutic counselling in a one-to-one or group setting and will help prepare the care partner for next steps and transitions in care when the time is right.
What you will be doing
- Managing a caseload of clients and providing timely follow-up as determined by the client’s needs and stated goals
- Building trusting relationship with clients
- Forming care plans with clients, with an equitable and client-centered approach to care
- Writing clear and concise case notes that address the client’s goals and plan of care
- Upholding client privacy in the collection, use and disclosure of personal health information
- Forming good working relationships with system partners to help clients navigate the health care system
- Organizing and facilitating group sessions
- Working in our offices, or other locations in the community between the hours of 8:30am to 4:30pm Monday to Friday; some flexibility may be required to meet client needs
- Taking part in an on-call rotation to cover services that run outside of regular office hours; you will be scheduled to carry the on-call phone for a week at a time, 6-7 times per year
What you will bring along
- You must have a post-secondary degree in Social Work from an accredited university with current registration with the Ontario College of Social Workers and Social Service Workers.
- The Society requires that new employees be fully vaccinated for COVID-19, subject to any accommodation obligations it may have under human rights legislation.

- Salary Range: $45,073 to $67,610

About the Alzheimer Society of Sarnia-Lambton
The Alzheimer Society of Sarnia-Lambton (ASSL) is a community support service organization that provides programs and services to people living with dementia and their care partners in Sarnia-Lambton. We work closely with other service providers to ensure quality care and support for our clients.
