We value your input on our First Link® Learning Series educational sessions so we can improve the way we help individuals living with dementia. The following questions ask about your experiences in our Learning Series sessions. We want to know what you found helpful as well as how we can improve this service.
Names will be held in the strictest confidence and only be used by the Evaluation Coordinator for tracking purposes.
