First Link® connects you to a supportive community of dementia experts, local services, and people who understand what you’re going through. Whether you are living with dementia, caring for someone who is, or seeking information, the Alzheimer Society is here to help—at any stage of the journey.

We support individuals and families in recognizing the signs of dementia and navigating the challenges that can come with a diagnosis. First Link® provides timely access to trusted information, guidance, and community-based supports.

How First Link® Can Help

Through First Link® you may be connected to:

  • One-on-one or group support services
  • Local health care providers and community resources
  • Peer connections with others in similar situations
  • Guidance on planning for the future and managing dementia

Accessing First Link®

Getting connected is easy—and no referral is required.

Anyone can self-refer to First Link® by calling our office at 807-345-9556. Whether you’re living with dementia, supporting someone else, or looking for guidance, our team is here to help.

We also welcome referrals from families, caregivers, health-care professionals, and community partners. With your permission, a health-care provider may refer you to our First Link® team, who will then contact you directly.

You don’t need to figure it out on your own. Our team can help you navigate next steps and determine the best supports for you.

Online: Book an appointment through our CareDove system. Our team will guide you through the referral process and help determine the best supports for you during your first appointment.

Referral Form: Families, caregivers, and professionals may also choose to complete a referral form. Our team will follow up directly once it is received.
