Alzheimer Society of Chatham-Kent (ASCK) is committed to protecting the privacy of information given by all individuals seeking to access ASCK services and websites. Personal health information (PHI) is collected by ASCK in accordance with the Personal Health Information Protection Act. Personal health information will not be disclosed outside of ASCK without your knowledge and consent, with only a few exceptions, such as where disclosure is required by law.

ASCK collects PHI in all forms (verbal, written, and electronic) for purposes related to direct client care and service delivery, administration and management of programs and services, client billing, administration and management of the health care system, research, teaching, statistical reporting, fundraising, meeting legal obligations and as otherwise permitted or required by law.
