The Alzheimer Society of Simcoe County connects people living with dementia and their care partners (family and friends) with our education and support services through our First Link® . We also connect people with health care professionals and community support services as needed. Anyone can make a referral to the Alzheimer Society of Simcoe County by calling our Barrie office or by faxing a completed First Link® referral form.

What is First Link®?


If you or your family member has been diagnosed with Alzheimer’s disease or another type of dementia, First Link® is for you. First Link® connects  individuals newly diagnosed with Alzheimer's disease and other dementias and their families to support early in the disease process. Support continues throughout the course of the disease.



First Link® will help you to:

  • Get connected early and support you throughout your journey
  • Learn about every step of the journey; from diagnosis to day-to-day living
  • Connect to Alzheimer Society programs, and other community and health care services

How to Make a First Link® Referral:

  • Call our Barrie office (705-722-1066) and complete the referral form by phone.
  • Fax the completed referral form to 1-705-722-9392.
  • You will receive an acknowledgment letter indicating the client's Family Support Coordinator.

  • To get connected to the Alzheimer Society early in order to prevent crisis situations
  • To reduce caregiver burnout and premature long- term care placement

Who can make a referral? 


Anyone can make a referral to First Link® including:

  • Self referral
  • Family physician
  • Family members
  • Community care partners
  • Allied health professionals
