Behavioural Supports Ontario (BSO) System Navigator

Overview

As a BSO System Navigator you will bring your social work skills to our team to make a difference in the lives of older adults experiencing responsive behaviours associated with dementia, mental health, addictions, and other neurological conditions. You will act as the primary link for BSO supports and assist care partners to navigate the healthcare system, providing them with ongoing support as needed.

You will work closely with a range of system partners and their BSO staff to establish continuity of care for clients through communication and collaboration. You will ensure clients living in the community and in long-term care have access to supports that meet their needs and reduce risk. This work involves triaging referrals, communicating with partners, and providing direct support to care partners through individual or group education, training, and counselling.

What you will be doing

  • Accepting and triaging client referrals and providing timely follow-up as determined by the client’s needs
  • Building trusting relationships with clients
  • Forming care plans with clients, with an equitable and client-centered approach to care
  • Writing clear and concise case notes that address the client’s goals and plan of care
  • Upholding client privacy in the collection, use and disclosure of personal health information
  • Forming good working relationships with system partners to help clients navigate the health care system
  • Organizing and facilitating group sessions
  • Working in our offices, or other locations in the community between the hours of 8:30am to 4:30pm Monday to Friday; some flexibility may be required to meet client needs
  • Taking part in an on-call rotation to cover services that run outside of regular office hours; you will be scheduled to carry the on-call phone for a week at a time, 6-7 times per year

What you will bring along

  • You must have a post-secondary degree in Social Work from an accredited university with current registration with the Ontario College of Social Workers and Social Service Workers.
  • The Society requires that new employees be fully vaccinated for COVID-19, subject to any accommodation obligations it may have under human rights legislation.

  • Salary Range: $45,073 to $67,610

The Alzheimer Society of Sarnia-Lambton (ASSL) is a community support service organization that provides programs and services to people living with dementia and their care partners in Sarnia-Lambton. We work closely with other service providers to ensure quality care and support for our clients.
