First Link Coordinator
At the Alzheimer Society of Windsor and Essex County, we are committed to supporting individuals and families affected by Alzheimer’s disease and other dementias through compassionate care, education, advocacy, and community support.
We are currently seeking a First Link Coordinator to join our team in an important front-line role supporting individuals living with dementia and their care partners across Windsor & Essex County.
About the Role
- The First Link Coordinator serves as the first point of contact for new clients and families seeking support. This role is responsible for:
- Managing incoming referrals and completing intake processes
- Providing education, information, and supportive guidance
- Conducting early-stage needs identification and triaging services
- Connecting clients and care partners to internal programs and community resources
- Supporting system navigation and care coordination
- Maintaining accurate and confidential client documentation

What We’re Looking For
- We are seeking candidates who have:
- A diploma or degree in social services, health, gerontology, or a related field
- Experience supporting individuals living with dementia and/or their families
- Strong communication, empathy, and crisis response skills
- Experience with intake, assessment, or care coordination
- The ability to work independently and collaboratively within a multidisciplinary environment
- Knowledge of dementia care and community resources
Additional Information
- 30 hours a week
- Monday to Friday, 8:30 am to 4:30pm
- Occasional evenings/weekends as required
- In-office position in Windsor-Essex County
- Remote work is not available
