We are here to help

Our staff and volunteers provide support programs, educational resources, referral services and more to ease the burden of care and improve the quality of life for people living with Alzheimer's disease and other dementias, as well as their care partners and families.

Experience has shown that the earlier people begin to learn and strategize about living with dementia, the better they are prepared. Earlier preparation can also help care partners and families provide support as the disease progresses.

Support programs in Huron and Perth Counties

The Alzheimer Society Huron Perth provides our registered clients with support through our First Link® Program. Click the link below to find information on our support groups and individual or group counselling options. Our aim is to provide a safe place to share information, thoughts, feelings and experiences.

We also help people find programs and services they need, such as day and respite programs, home support and help with the difficult transition to long-term care.

Dementia Education in Huron and Perth Counties

Education is an important part of understanding and learning to live well with dementia. We provide education for many people whose personal or professional lives are impacted by dementia. 

Our Education Hour page offers a listing and direct registration to all or our upcoming free online webinars. These are open to anyone - we look forward to seeing you there!

Social Recreation in Huron and Perth Counties

Social recreation is a significant aspect for quality of life. Our social recreation team has many opportunities to get involved and stay connected. 

Minds in Motion®

Through Minds in Motion®, people living with dementia will experience meaningful and beneficial socialization and stimulation through a number of appropriate activities. This program is open to registered clients.

In-Home Recreation

The In-Home Recreation Program offers individualized activities for persons living with dementia either in-home or virtually. Following a recreation and leisure assessment, the coordinator will develop an individualized plan based on interests.

The In-Home Recreation Coordinator or a carefully selected and trained volunteers will facilitate activities and provide social and cognitive stimulation to the client through ongoing weekly visits. 

This program also aims to empower care partners by demonstrating activities and providing educational opportunities and support.

This program is open to registered clients.

Safety services

When Alzheimer's disease and other dementias progress, the person living with the disease has the potential to go missing. This situation presents a serious safety concern but know that help and support is available.

Finding Your Way®

Finding Your Way® is a website dedicated to helping people living with dementia, their families, caregivers and communities to recognize the risk of going missing, be prepared for incidents of going missing, and ensure that people with dementia can live safely in the community.

Referral Forms

If you are a Healthcare professional and would like to make a referral, please:

  1. Ask your patient or their family member’s permission to forward their name to the Alzheimer Society.
  2. Then, fill out the First Link® Referral Form and return it to the Alzheimer Society Huron Perth. Find out more about our First Link® Program here.

Self Referral

If you are concerned that you may have dementia or have been recently diagnosed and are unsure what your next step is, we can help you get the information you need.

Remember, dementia is not a normal part of aging.

  • If you are concerned about any of the early warning signs, talk to your doctor.
  • You know your body and you should speak up if you have worries about Alzheimer’s disease or other dementias.
