Referring to First Link®

When you refer someone to First Link®, you are connecting them to a community of information, support, and services for people living with dementia and their care partners.

First Link® is a program that connects people with dementia and their families to Alzheimer Society programs and other community services as early as possible and throughout the progression of the disease. First Link clients receive individualized support and regular scheduled follow-up calls from our First Link Coordinators.

Direct referrals are made by health care professionals and community service providers. See below for referral information.

Individuals can self refer via our Dementia Helpline.

Once a referral is made, an Alzheimer Society trained staff member connects with the person with dementia and their family members to link them to Alzheimer Society services and other programs and services within the community.

Who can refer?

  • Physicians, geriatric specialists, and memory clinics
  • Health providers including: nurse practitioners, nurses, social workers, occupational therapists, pharmacists, and others
  • Community organizations working with individuals and families affected by dementia

When to refer?

Any time during the dementia journey.

How to refer?

  1. Ask the individual or family member for consent to forward their name to First Link®
  2. Complete the one page direct referral form (see below) and return to the Alzheimer Society of Saskatchewan via fax 1-866-746-1507 or email: [email protected]
