Improving hospital-to-home transitional care to help older people recover after hospital discharge
Invitation to Participate: You are invited to participate in this research study because you are being/were recently discharged from the hospital, or you are helping your relative after a hospital stay.
Participation: You will be asked to give your verbal consent for a research assistant to contact you to determine if you are eligible to participate. You will be asked some questions to determine if you are eligible to participate. If you are eligible, you will be asked to verbally consent to participate in the study. If you consent, you will be interviewed by a Research Assistant by videoconference or by telephone, or in person in the hospital or at your home. You can choose to have the interview in two parts. The interview will ask you about common problems people have with their physical and mental functioning after coming home from hospital and your opinion on strategies that might help the problems. The problems and strategies will be described to you using pictures to help you understand them. You or your family or friend will be asked demographic and health related questions on your behalf such as your age and reason for hospital admission. The interview will take up to 60 minutes. Your responses will be audio-recorded.
