Memory Testing and Cognitive Assessments

Are you a family member or care partner of someone living with dementia, and curious about your own cognitive health? 

ASDR is offering cognitive assessments to help you understand any changes in your memory or thinking abilities. These assessments can identify early signs and provide strategies to support both your brain health and that of your loved one. 

Those who identify concerns with memory loss, difficulty thinking, problem-solving, language, or other cognitive issues should consider getting a cognitive assessment.  

Eligibility:  

  • Over the age of 18 
  • Have memory or cognitive concerns
  • Complete consent, demographics and related questionnaires
  • Be willing to complete a cognitive assessment
  • Must have a family doctor or primary care provider and be willing to meet with them to receive the results
  • Have not received a cognitive assessment within the last 6 months
