Advance Care Planning is the process of thinking about your wishes and directions for healthcare and end-of-life situations and communicating those wishes to the person/people who will make decisions for you if you are not able.
This webinar will help provoke thought and conversation about the importance of Advance Care Planning, the role of a Substitute Decision Maker, and tools for creating an Advance Care Plan.
We will be joined by Diane Gwartz, Nurse Practitioner and Niagara Community MAiD Assessor & Provider, who will be discussing Medical Assistance in Dying (MAiD).
