The journey continues for persons living with dementia and their care partners following the move to long-term care. Families often face a significant period of adjustment as their loved one settles into a care facility.
Join us as we celebrate the Alzheimer Society Niagara Region’s receipt of an Ontario Trillium Foundation Grant to support the Long-Term Care Transitional Support Service. This service is designed to support family care partners in their adjustment through individual counselling and support groups.
MPP Jennie Stevens will join us for the recognition of this grant, followed by our public forum with guest speaker Dion McParland discussing:
- Meaningful and Engaging Visits
- Self-Health and Re-establishing a Sense of Self
- New Roles and Responsibilities
- Guilt, Grief and Loss
- Building Support Networks in the LTC Community
