In this presentation, Dr. Gloria Puurveen will highlight the development of a volunteer-based navigation program called Nav-CARE that provides support for people living with declining health and their families to live well to the end of life in the community. She will share an overview of the Nav-CARE toolkit, insights from research to evaluate the program and how her team is developing Nav-CARE to support people living with dementia and their families.
